1. Program name and operator
The text messaging program described here is operated by SLB Healthcare, LLC, a Georgia limited liability company doing business as SLB In-Home Care (“SLB Healthcare,” “we,” “us,” or “our”). Our messages identify us as SLB Healthcare.
Business address: 4751 Best Road, Suite 309, Atlanta, GA 30337. Phone: (470) 570-7053. Email: info@slbinhomecare.com.
2. Program description — what we send
This is a conversational and notification-based messaging program for families, patients, and authorized representatives who are enrolled with, or in the process of enrolling with, SLB Healthcare, LLC for in-home pediatric nursing services. Messages you may receive include:
- Reminders and confirmations for scheduled nursing visits, RN assessments, and in-home appointments
- Scheduling changes, nurse-arrival notifications, and shift coverage updates
- Intake, eligibility, and paperwork follow-up (for example, a missing physician order or Medicaid document)
- Care coordination messages from your assigned RN case manager
- Replies to questions you send to us by text
- Service and operational notices, such as an office closure or an inclement-weather schedule change
We do not send marketing, promotional, or advertising text messages. We do not send messages relating to loans or lending, cryptocurrency, gambling, cannabis, CBD, tobacco, vaping, alcohol, firearms, or adult content. We do not send messages on behalf of any other company, and we do not text people who have not given us their express written consent.
3. How you opt in (consent)
You will only receive text messages from us if you have given us your prior express written consent. Consent is obtained in one of the following ways:
- Website form. You check the SMS consent box — which is never pre-checked — on our contact page and submit the form with your mobile number.
- Intake and admission packet. You check and initial the SMS consent box on the paper or electronic intake, admission, or service agreement documents that a nurse or intake coordinator reviews with you.
- Inbound text. You text us first at our business number, or reply START or YES to a message from us.
- Verbal consent, documented. You give consent during a recorded or documented phone call, and we record the date, time, number, and language used in your file. Where verbal consent is used, we send a confirmation text describing the program, frequency, rates, and how to stop.
Consent is not a condition of receiving care, services, or any purchase. You can decline text messaging and still receive every service we offer; we will simply reach you by phone or email instead.
4. Message frequency
Message frequency varies depending on how active your child’s case is. Most families receive approximately 2 to 8 messages per month. Families in active intake, or with frequently changing shift schedules, may receive more. Conversational replies you initiate are not limited.
5. Cost — message and data rates
SLB Healthcare, LLC does not charge you for participating in this program. However, message and data rates may apply and are billed by your mobile carrier according to your plan. Please contact your carrier if you have questions about your plan or charges.
6. How to opt out (STOP)
You may cancel text messages at any time. Reply STOP to any message you receive from us. You may also reply END, CANCEL, UNSUBSCRIBE, or QUIT.
After you send one of these keywords, we will send you a single confirmation message and then stop sending texts to that number. For example:
“SLB Healthcare: You have been unsubscribed and will receive no further text messages from us. For help, call (470) 570-7053. Reply START to re-subscribe.”
Opting out of text messages does not cancel your services or affect your child’s care in any way. We will continue to reach you by phone and email. You may also opt out by calling us at (470) 570-7053 or emailing info@slbinhomecare.com. If you want to start receiving texts again, reply START to the same number or contact our office.
7. How to get help (HELP)
Reply HELP to any message for assistance. You will receive a reply such as:
“SLB Healthcare in-home pediatric nursing. Msg freq varies. Msg&data rates may apply. Reply STOP to cancel. Help: (470) 570-7053 or info@slbinhomecare.com”
You can also reach us Monday through Friday, 9:00 AM to 5:00 PM Eastern Time at (470) 570-7053 or info@slbinhomecare.com.
Text messaging is not for emergencies. Do not use text messaging to report a medical emergency or an urgent change in your child’s condition. Messages are not monitored continuously and may be delayed. If your child is having a medical emergency, call 911. For urgent clinical issues, call our 24/7 on-call nurse line at (470) 570-7053.
8. Supported carriers and delivery
This program is available on most major U.S. wireless carriers, including AT&T, T-Mobile, Verizon Wireless, Sprint, Boost, Cricket, MetroPCS, U.S. Cellular, Virgin Mobile, and others.
Carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission by your wireless carrier and is not guaranteed by SLB Healthcare, LLC. We are not responsible for messages that are delayed, undelivered, or misdirected due to carrier issues, device settings, number changes, or network conditions.
9. Privacy and your mobile information
Our full Privacy Policy explains how we collect, use, and protect your information.
Mobile information sharing
No mobile information will be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except with vendors and subcontractors acting on our behalf strictly to deliver the messaging service (for example, our SMS platform provider and telecommunications carriers), who are contractually prohibited from using that information for any other purpose.
10. Protected health information and SMS security
SLB Healthcare, LLC is a HIPAA-covered entity. Standard SMS text messaging is not a secure or encrypted channel. Anyone with access to your phone, or to your mobile carrier account, may be able to read messages sent to your number.
Because of this, we deliberately limit the protected health information (PHI) included in text messages. Our texts are generally limited to scheduling, logistics, and requests to call the office. We will not include detailed clinical information, diagnoses, or test results in a text message unless you specifically ask us to and acknowledge the risk.
By opting in, you acknowledge that you understand SMS is not secure, and you consent to receive the limited health-related information described above by text at the number you provided. You may withdraw this consent at any time by replying STOP or contacting our office.
11. Eligibility and your responsibilities
- You must be at least 18 years old, or the parent or legal guardian of the child receiving services, or an authorized representative, to enroll in this program.
- You must be the subscriber of, or have authority over, the mobile number you provide.
- You are responsible for notifying us if your mobile number changes or is reassigned. If you give up a number, please tell us so we do not text the next person assigned that number.
- Some devices and plans may not support all message types. Message content may be truncated on some handsets.
12. Changes to these terms
We may update these SMS Terms & Conditions from time to time. The current version, with its effective date, will always be posted on this page. Material changes to the messaging program will be communicated to enrolled participants. Your continued participation after an update constitutes acceptance of the revised terms.
