SLB Healthcare, LLC provides in-home skilled nursing and personal care support for children in Georgia who depend on ventilators, tracheostomies, feeding tubes, oxygen, and other complex medical technology. For families who qualify through Georgia Medicaid’s GAPP program, care is provided at no out-of-pocket cost.
Who we care for
Children from birth through age 20 who are medically fragile and need hands-on nursing at home rather than in a hospital or facility.
- Ventilator and tracheostomy dependence
- G-tube, J-tube, and NG feeding
- Seizure disorders requiring rescue medication
- Oxygen therapy, suctioning, and pulse-ox monitoring
- Complex medication schedules
- Total dependence for activities of daily living
What we do
Every family is assigned a Registered Nurse case manager who builds the plan of care with you, coordinates with your child’s physician, and stays reachable.
Skilled Nursing (RN & LPN)
Licensed nurses in the home for shifts that fit your family — days, nights, weekends, and overnights. Vent and trach management, tube feeding, wound care, medication administration, and continuous assessment.
Personal Care Support (PSS)
Trained caregivers — including, in many cases, a parent or family member who can be hired and paid — to assist with bathing, dressing, feeding, toileting, transfers, and positioning.
RN Assessment & Care Planning
A comprehensive in-home nursing assessment, a written plan of care, and documentation submitted on your behalf to support the hours your child clinically needs.
GAPP Eligibility Help
We walk families through Medicaid and GAPP enrollment, gather physician orders, and handle the paperwork — at no charge, whether or not you ultimately choose us.
Care Coordination
We coordinate between your child’s pediatrician, specialists, DME suppliers, school, and Medicaid care management organization so nothing falls through the cracks.

Phone, email, and — for families who opt in — text message updates about visit scheduling, nurse arrival, and care coordination. See SMS Terms.
How it works
- Reach out Call, email, or send the form on our contact page. Tell us about your child.
- Free eligibility review We check Medicaid and GAPP eligibility and tell you honestly whether your child is likely to qualify.
- In-home RN assessment A Registered Nurse visits, assesses your child, and writes the plan of care with you.
- Physician orders & approval We prepare the documentation, obtain physician signatures, and submit for authorization.
- Care begins We staff your case with nurses matched to your child’s needs and your family’s schedule.

Families who are enrolled with us — or who are going through intake with us — can choose to receive text messages from SLB Healthcare, LLC about their child’s care. This is entirely optional. We only text people who have asked us to, and you can stop at any time.
What our text messages are used for
- Confirming and reminding you about scheduled nursing visits and RN assessments
- Letting you know when a nurse is running late or a shift has changed
- Following up on intake, eligibility, and paperwork we still need from you
- Care coordination messages from your assigned RN case manager
- Replying to questions you text us
We do not send marketing or promotional text messages. Message frequency varies, and is typically about 2–8 messages per month depending on how active your child’s case is. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for help. Consent to receive text messages is never required to receive care from us.
