Private-duty skilled nursing (RN and LPN)

Skilled nursing is hands-on care that legally must be performed by a licensed nurse. Our RNs and LPNs work shifts in your home — commonly 8, 10, or 12 hours — so your child can live at home instead of in a hospital or long-term care facility, and so caregivers can sleep, work, and care for their other children.

Typical skilled interventions we provide:

• Respiratory: ventilator management, tracheostomy care and tube changes, suctioning, oxygen administration, nebulizer treatments, chest physiotherapy, pulse oximetry monitoring
• Nutrition: G-tube, GJ-tube, J-tube, and NG feeding; pump management; site care; hydration monitoring
• Neurological: seizure monitoring and documentation, administration of rescue medications, VNS magnet use, neuro checks
• Medication: administration by all routes, including scheduled, PRN, and emergency medications; monitoring for adverse effects
• Assessment: continuous clinical assessment, vital signs, intake and output, skin integrity, and documentation for the physician and payer
• Emergency response: recognizing and responding to respiratory distress, aspiration, status epilepticus, and equipment failure

Personal Care Support (PSS)

Personal Care Support covers hands-on assistance with activities of daily living that a child cannot perform independently because of their medical condition. Under the Georgia Pediatric Program, a parent or family member can in many cases be hired, trained, and paid to provide these hours.

• Bathing, grooming, oral care, and skin care
• Dressing, including around trach ties, G-tube sites, braces, and lines
• Feeding assistance and mealtime supervision
• Toileting, diapering, and incontinence care
• Transfers, lifting, positioning, and repositioning to prevent skin breakdown
• Mobility assistance and range-of-motion support
• Supervision and safety monitoring

RN assessment and plan of care

Before care begins, a Registered Nurse conducts a comprehensive in-home assessment of your child. That assessment produces a written plan of care listing every problem, the interventions we will perform, and measurable goals — the document your child’s physician signs and the payer reviews.

We prepare and submit this documentation on your behalf. If a request for hours is reduced or denied, we help you understand why and what additional clinical documentation would support an appeal.

Georgia Pediatric Program (GAPP) eligibility assistance

GAPP is a Georgia Medicaid program that pays for in-home skilled nursing and personal care support for medically fragile children from birth through age 20. Because it is a Medicaid benefit, qualifying families receive care at no out-of-pocket cost.

Broadly, a child may qualify if they are Medicaid-eligible in Georgia, are under 21, and have a medical condition requiring care that would otherwise be delivered in a hospital or facility. Eligibility is determined by the State, not by us — but we will review your child’s situation honestly and free of charge, help you gather physician orders and records, and complete the enrollment paperwork.

Care coordination and family communication

Each family is assigned an RN case manager who is the single point of contact for scheduling, clinical questions, supply issues, and coordination with your pediatrician, specialists, DME company, school nurse, and Medicaid care management organization.

We communicate with families by phone and email by default. Families who prefer text messaging can opt in to receive SMS updates about visit scheduling, nurse arrival times, and care coordination. Service area and hours

We serve families throughout metro Atlanta and surrounding Georgia counties. Our office is open Monday through Friday, 9:00 AM to 5:00 PM Eastern Time. A nurse is available on call 24 hours a day, 7 days a week for active clients.
